A sore heel that makes the first steps of the morning miserable, or an aching elbow that flares every time you lift at work, can make a quick solution sound very appealing. When comparing shockwave therapy vs cortisone injections, the best choice is rarely about which treatment is “stronger”. It is about the source of your pain, the condition of the tissue, your goals, and the plan needed to help you return to comfortable movement.
Both treatments can have a place in musculoskeletal care. They work in very different ways, though, and neither should be treated as a stand-alone fix for every tendon, joint or soft-tissue problem.
Shockwave therapy vs cortisone injections: the key difference
Shockwave therapy uses acoustic pressure waves delivered through the skin to a targeted area. In physiotherapy, it is commonly considered for persistent tendon-related conditions, including plantar heel pain, Achilles tendinopathy, tennis elbow and some shoulder tendon problems. The treatment aims to stimulate a healing response in tissue that has become painful, less tolerant of load, or slow to recover.
A cortisone injection is a medical procedure administered by a qualified doctor. Cortisone is a corticosteroid medication that may reduce inflammation and pain in selected conditions. It is often injected into or around a joint, bursa or tendon sheath, depending on the diagnosis and the clinician’s assessment.
In simple terms, cortisone is usually intended to calm symptoms relatively quickly, while shockwave therapy is generally used as part of a longer-term rehabilitation approach for certain chronic tendon conditions. Neither option replaces an accurate diagnosis, sensible activity modification and a tailored exercise program.
When shockwave therapy may be considered
Shockwave therapy is not designed for every painful area. It tends to be most useful where symptoms have persisted despite appropriate early management, particularly when a tendon or the plantar fascia is involved. Tendons need to tolerate force. If they are repeatedly overloaded or have not rebuilt capacity after an injury, pain can continue even when rest provides temporary relief.
For example, a person with plantar heel pain may notice sharp discomfort on their first few steps, then an ache after long periods standing or walking. Someone with lateral elbow pain may struggle to grip tools, carry shopping bags or play tennis. In these situations, shockwave therapy may be considered alongside progressive strengthening, changes to aggravating activities, footwear advice where relevant, and hands-on physiotherapy treatment.
Treatment is usually delivered across a series of appointments rather than as a one-off procedure. Some people feel temporary soreness during or after a session, similar to the discomfort that can follow a challenging exercise session. Improvement is often gradual, and results vary depending on the condition, how long it has been present, general health, load demands and follow-through with rehabilitation.
Shockwave therapy may not be suitable for everyone. Your physiotherapist will screen for factors such as pregnancy, bleeding conditions or medications, certain nerve or circulation issues, infection, malignancy in the treatment area, and other medical considerations. A careful assessment matters because heel, shoulder and elbow pain can have several possible causes.
When a cortisone injection may be considered
Cortisone injections can be helpful when pain and inflammation are significantly limiting function, sleep or participation in rehabilitation. They may be considered for conditions such as an inflamed bursa, some arthritic joints, or particular inflammatory presentations where a doctor considers the likely benefits to outweigh the risks.
For some patients, reduced pain can create a valuable window to begin movement and rehabilitation. If a painful shoulder is preventing sleep and making basic tasks difficult, for instance, an injection may help settle symptoms enough for the person to engage more comfortably with their exercise program.
However, the effect is not always long lasting. Cortisone can reduce pain without necessarily resolving the mechanical, strength or load-related factors that led to the problem. Returning immediately to heavy work, repetitive overhead tasks or sport without rebuilding capacity can allow symptoms to return.
Cortisone also needs particular caution around tendons. Repeated corticosteroid injections in or close to some tendons may affect tissue quality and can increase the risk of tendon damage or rupture. The risk depends on the location, dosage, technique, number of injections and individual circumstances. This is one reason a doctor and physiotherapist may recommend a more conservative rehabilitation plan, or consider shockwave therapy, before or instead of an injection for certain tendinopathies.
Possible short-term effects of an injection include post-injection pain flare, skin colour changes, temporary changes in blood sugar levels for people with diabetes, and infection, although serious complications are uncommon. Your doctor should explain the expected benefit, alternatives and risks for your individual situation.
Relief now versus rebuilding for later
The decision is often less about choosing one treatment forever and more about identifying the right sequence of care. A cortisone injection may be useful when inflammation is dominating the presentation and pain is preventing normal function. Shockwave therapy may be a more relevant option when a persistent tendon problem needs a gradual stimulus for recovery.
There are also times when neither is the first step. Recent injuries, acute swelling, fractures, nerve-related pain, referred pain from the spine, and some joint problems require different assessment and management. A painful heel is not automatically plantar fasciitis, just as every sore shoulder is not a bursitis problem.

A physiotherapy plan can address this with appropriate strengthening, mobility work, hands-on treatment where indicated, education about pacing, and a realistic return-to-activity plan. For tendon pain, exercises are usually progressed over time to help the tissue tolerate the demands of work, sport and everyday life. The right level of loading matters: too little can leave the area deconditioned, while too much too soon can keep it irritable.
This is also why rest alone often falls short for persistent tendon pain. Reducing aggravating activity can settle a flare, but the underlying tissue still needs a gradual pathway back to capacity. Your program should fit your starting point, whether you are recovering after surgery, managing a workplace injury, living with chronic pain, or trying to stay active around family commitments.
Questions worth discussing at your appointment
Before deciding on shockwave therapy or an injection, ask what structure is likely causing your pain and whether the diagnosis has been confirmed clinically. It is also reasonable to ask what improvement is realistic, how long it may take, what side effects to watch for, and how the treatment will work alongside exercise and day-to-day activity changes.
If you are considering a cortisone injection, discuss whether the area involves a tendon, whether repeat injections are advisable, and what you should avoid immediately afterwards. If shockwave therapy is suggested, ask how many sessions may be appropriate and what rehabilitation you will need to complete between appointments.
The clearest treatment pathway is one that accounts for your symptoms and your life, not just an image scan or a single painful spot. With an accurate assessment and consistent rehabilitation, the goal is not simply to quieten pain for a few days, but to help you move with greater confidence in the weeks and months ahead.
